Joe--have never seen either of your afore mentioned ministries in the Passaic Eparchy, though I am isolated. ;-)
My sons went to the Passaic altar server weekend camp last year. But, even there, they did not get any specific prayers regarding preparation for DL or other liturgical services or to say as they vest. Sounds like a real program is lacking all around.
Did you contact the seminary to see if they have anything? Or your chancery? Up for the task of composing one yourself? ;-)

I am only sorry that, as an altar-server, I was not taught to ask for Father's blessing etc.
Is this practice wide-spread in the Byzantine Catholic Church?

Joe, I mentioned your post that there were vested female altar servers in the Eparchy of Parma to my Pastor. He was very surprised. He was even more surprised when I told him that, according to your post, Mary-Martha severs were being promoted by our Eparchy(Parma). Here at St. Georges it is used definately and I never noticed it up home. It is truly wonderful to see the servers approach Father with their vestments folded in their hands for his blessing to serve. As he blesses them, then of course the kiss his hand. So that in itself is truly a prayer.

Here is a prayer for Latin Rite altar servers from England, authorised by the Archconfraternity of St Stephen (for altar servers).
"O Lord, I have many interesting things that I want to do today, but please help me to remember that I am assisting at this Mass. Help me to understand that if I serve with dignity and reverence it will help the people to pray; if I serve poorly it will distract them. Help me to serve well in the name of Jesus our Lord. Amen."
